ASSISTED PLAN:

The monthly residence fee includes

• One hour per day of combined personal care service and medication management

• Limited escort service for medical visits

• Spacious private studio apartment

• Heat, electricity, water and basic cable

• Three meals served daily

• On site staff 24 hours a day

• Security monitoring system

• Weekly housekeeping, linen and personal laundry service

• Planned social and recreational events

Studio Apartment: $2,400 (per month)

INDEPENDENT PLAN:

The monthly residence fee includes

• Spacious private studio apartment

• Heat, electricity, water and basic cable

• Three meals served daily

• Security monitoring system

• Weekly housekeeping, linen and personal laundry service

• Planned social and recreational events

Studio Apartment: $1,850 (per month)

BASIC SERVICE PLAN:

The monthly residence fee includes

• Gross monthly income ($100 left for resident spending money) Single Studio Apartment or Shared Studio Apartment; private or shared bath.

• Three meals served daily*

• Weekly housekeeping of unit - dusting, vacuuming, total and complete cleaning of bathroom.

• Laundry one time per week, washed and folded (up to 14 lbs)

• All utilities (excluding telephone service)

• Basic cable - $5.00 monthly

• Ground and Facilities maintenance

• Social and recreational programs

• Monthly wellness program

• Access to Registered Nurses for information and referral

• Full time licensed Social Worker

• 24 Hour emergency Response System with 24 hour awake, alert staff

• Electronic monitoring system with door contacts, video camera with intercom system

OPTIONAL SERVICES:

• Personal Care Assistance (GAFC covers)
Assist with activities of daily living (i.e. bathing, dressing, grooming, toileting, ambulation or other specific personal care needs.) A care plan outlining assistance done by a Registered Nurse. All personal care staff are highly trained. Orientation and supervision of personal care staff is done by a Licensed Nurse.

• Self-Administered Medication Management Program (GAFC covers) Personal Care Staff will provide Self-Administered Medication Management regarding medication regime as outlined in the care plan and Self-Administered Medication Policy.
